Your Role And Responsibilities
As a Hospice RN Case Manager, your compassion and clinical knowledge will guide every patient interaction. You’ll be responsible for:
- Holistic Care Planning: Conduct physical, emotional, and environmental assessments to develop individualized care plans.
- Direct Patient Care: Deliver hands-on nursing care that supports comfort and quality of life.
- Team Communication: Coordinate with physicians, supervisors, and interdisciplinary team members to align care strategies.
- Family Education: Support patients' loved ones by offering resources, guidance, and compassionate communication.
- Proactive Monitoring: Identify and respond to changes in patient condition to ensure timely, appropriate interventions.
- Excellence in Care: Adhere to all physician orders, care plans, and regulatory requirements with professionalism and respect.
- Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s)
- Current RN license in Michigan
- Minimum of 1 year of clinical nursing experience
- 1 year of Hospice or Home Health experience required
- Reliable transportation for home visits
- Physical ability to sit, stand, bend, lift (up to 50 lbs), and move throughout the day
- Strong organizational skills and the ability to document thoroughly and meet deadlines
