Position Summary

Oversees and coordinates care for specific specialty area for an assigned caseload. Facilitates appropriate care, assists patients and families in finding and negotiating that care. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and cost effective care.

What You Will Do

  • Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status. Includes assessment of past and present treatment course. Able to understand and develop goals, treatment plan and provider options.
  • Collaborates with other clinical teams and medical staff in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for identified populations.
  • Serves as clinical expert related to populations served providing education and internal consultation to other staff.
  • Utilizes critical thinking and problem solving skills to analyze and synthesize all data and formulate an appropriate case management plan that addresses the patient’s individual and family needs.
  • Monitors and modifies the plan of care through an interdisciplinary and collaborative process in conjunction with all parts of the health care team.

What You Will Need

  • Graduation from an accredited Nursing school program
  • Associates Degree in Nursing required, BSN preferred.
  • In Grand Rapids if ADN must enroll in BSN program within 6 months of hire and complete in 4 years of hire
  • Licensed in the State of Michigan
  • Must be Chemo/Biotherapy Certified
  • 1 years of experience as an RN required if new grad must apply to RN residency that takes place three times a year in February, July and October
